Purpose Driven Elite 7U Youth Ministry Program Registration

This registration fee provides participation in the Purpose Driven Elite Youth Ministry Program and includes the equipment and resources needed for the season.

Registration Includes:

• 3 Personalized Game Uniforms — Home (Gold), Away (Black), Alternate (White)

• 1 Personalized Wooter Backpack

• 1 Conditioning Outfit (Shirt + Shorts)

• 1 Team Towel

• Participation in Purpose Driven Elite Youth Ministry Program Activities

• End-of-Season Banquet

• Registration Fee is non-refundable once uniforms have been ordered.

Customization:

• All three uniforms include the player’s selected name and number.

• Backpack includes the player’s last name and selected number.

• Conditioning shirt features the player’s number on the back and the team logo on the front.

Please enter the player’s last name exactly as you want it printed and select the player’s jersey number along with all required sizes.
